Need help with NDA Agreement

I hope everyone is doing well, i need some help from you PROS.

I am about to sign an NDA and it is my first time doing so and really confused about some small and minor stuff. I would like to ask your guidance on the matter. i am adding a part of the contract i need help with numbers.

i need to fill this contract as Individual Freelancers so how do i handle point 1,2,3

also, point 5 and 6. Disclosee and Attention both should be on my name?

really confusing stuff for me. any help guidance much much appreciated.

If you work through a company you need to write the name of your company in 1, and the country it is registered in 2, and the company office address in 4. Then write your company name in 5 with your company address also. Then write your own name and email in 6. If you do not work through a company then leave 1 and 2 empty and write your name in 3 and your address (not a PO Box) in 4. Put your name and address in 5 and your name and email in 6. That should work.
